What is the Primary Care Mental Health Team?

The Primary Care Mental Health Team is a specialist team working for our network of GP practices to support people with their mental health and wellbeing. We work alongside GPs and other primary care staff to provide timely assessment, advice and support for people experiencing mental health difficulties.

Our team includes mental health professionals who can offer holistic assessments, brief interventions, medication reviews, and help you access the right support services. We focus on early intervention, recovery and helping people manage their mental health closer to home, reducing the need for specialist services where possible.

We work closely with GPs, social prescribers, care coordinators and community mental health services to ensure joined-up, person-centred care tailored to your individual needs.
