Yog is a clinical pharmacist who has worked for St Helen south PCN since March 2021. Yog graduated over 14 years ago from Liverpool John Moores University with a MPharm degree and has worked across a number of settings both NHS and non-NHS in that time. Yog started his career in community pharmacy and made his way to his current role in the NHS. He then went back to LJMU in 2018 to study for a Clinical Diploma in Primary and Integrated care with a Prescribing qualification, which was completed in March 2020. And then undertook the NHS funded Primary Care pathway.
Yog – ‘I feel this journey has given me a better understanding of the patient experience of accessing care and it helps me in my current role to promote more ways to work towards better access. I lead a team of 3 pharmacist and 3 technicians who work across the St Helens South area. We review our focusses regularly based on my teams journey of developing their skills such as becoming Non-medical prescribers or achieving additional clinical skills which would help set up our involvement in vaccination, triaging, managing long term conditions and educational sessions.’
‘Our workplan is focussed on delivering structured medication reviews to the patients covered by the network practices and also reviewing our care home residents. Our reviews cover a broad spectrum from medical management to a more holistic approach depending on our patient needs. Therefore, it also means we work collaboratively with all the teams linked to our networks if patients need additional support, even our community pharmacy colleagues.’
