Lauren gained a Bsc Adult Nursing in 2008, Bsc Midwifery 2013 and has been an Independent Non Medical prescriber for 10 years with extensive experience in the identification and prescribing of polypharmacy. Lauren has a Msc Advanced Practice from Edge Hill University and has recently completed a Masters module in the Identification and Management of Complex Frail patients. Lauren is a member of the British Geriatric Society and is both a Generic and Advanced Life Support Instructor for the Resuscitation Council.
Lauren has worked within community for the last 10 years supporting our complex elderly and frail population. She is vastly experienced in caring for patients and their families with complex long term conditions and varying degrees of frailty. Lauren is fiercely passionate about the care of our elderly population ensuring comprehensive assessments are complete to ensure patients are educated, listened too and well cared for within their homes, with their needs and wishes addressed including hospital avoidance, future care planning and palliative care.
As the Frailty Nurse Lead, Lauren is respectful and adaptable to this wonderful cohort of patients and along with her team of advanced nurses, frailty pharmacist and skilled clinical care co-ordinators, she leads the team for the PCN and our member practices linking in at MDTs and educating staff around the management of frailty and sharing learning.
Lauren alongside the frailty team work collaboratively within the PCN network, with occupational therapists, GP colleagues, practice nurses and social prescribers to help the frail population within the St Helens South PCN to enhance their quality of life, maintain independence, reduce unnecessary hospital admissions and supporting families and their carers.
