We are working with our member GP practices and local partners to develop a neighbourhood approach to health and care, bringing services and organisations together around the needs of our local communities.
Neighbourhood working is about making better use of the skills, knowledge and resources available across primary care and the wider health and care system. By working more closely together, we aim to make it easier for people to access the right support, improve coordination between services and respond to the health needs of our local population.
Working together for our communities
Our PCN is developing new ways of working with GP practices and partner organisations across our neighbourhood.
This includes looking at opportunities to:
- Bring services closer to the communities that need them
- Improve coordination between primary care and other local services
- Support earlier intervention and prevention
- Reduce unnecessary pressure on GP practices and other services
- Make better use of the skills and expertise available across our teams
- Develop services in response to local population health needs
Our Neighbourhood Projects
We are developing a number of Neighbourhood Projects in partnership with our member practices and local organisations.
Each project will focus on an identified need within our local population and explore how organisations can work together differently to improve access, outcomes and patient experience.
