What is a Social Prescribing?
Social prescribing plays a crucial role in supporting GPs and improving patient well-being.
It promotes a collaborative approach between healthcare professionals, community organisations, and patients.
By connecting patients with non-medical resources and community-based support, social prescribing addresses the social determinants of health.
This approach recognises that factors such as loneliness, housing, employment, and lifestyle choices significantly impact overall health outcomes.
How Social Prescribers support patients?
Social prescribers offer holistic approach that goes beyond traditional medical interventions.
They can signpost to a range of local services to support:
- Social, emotional and physical wellbeing
- Weight-loss programs, exercise classes, local activities and clubs
- Substance misuse, gambling
- Debt, housing advice and volunteering opportunities.
- Loneliness and elderly support groups
- Signposting to a range of charities and local organisations
Social prescribing empowers patients to take control of their health and well-being, fostering self-management and reducing reliance on medication.
